A flag was given to Jerry Kosik upon his retirement as Chief of Police in River Forest.  It is a mid-blue field, with a circular logo in white with yellow rings.  Around the perimeter are the words THE VILLAGE OF NORTH RIVERSIDE and 1923-1998, and two small NR logos.  In the center, '75TH Anniversary'.
River Forest is a suburb of Chicago.

This flag was presented to Jerry Kosik by the City Council on his retirement. He informed me at that time that a copy of the municipal flag is presented to various public officials either upon their retirement or the completion of their term of office, and in the case of those who died in office the flag is given to the surviving relatives. There is a large standing silk flag in this design on a brass indoor staff with a golden eagle at the top which stands inside the council chamber, and also at the right side of the Mayor's Office when one enters. This latter flag is in a group with the S&S, the Illinois state flag, and the county flag (I am not sure whether we are speaking of Cook, Lake, or Du Page Counties). When the Mayor dies in office, retires, or is defeated in an election, he receives this North Riverside flag in the same dimensions as the silk flag, but in cotton and not mounted. Senior public officials such as the Chief of Police, Fire Chief, and Department Heads receive the same flag upon their retirement or leaving office for whatever reason. Members of the City Council have this flag in a small artificial silk version on their desks in the Council Chamber, and they take this with them when they leave office. However, the North Riverside flag does not fly outside any public buildings in the town. Rather elaborate flag protocol for a small bedroom suburb of Chicago!

I don't know if the 75th Anniversary flag has permanently supplanted the original flag, nor if the same protocol is/was used with respect to this flag.
